Coaching Reimbursement

******Please ensure that Step 8 of the coaching steps is completed before submitting for reimbursement****** Checks will not be issued if we do not have the "Code of Conduct" form and the "coaching agreement" form on file.

To request reimbursement for required USA Hockey Registration and Training please fill out the Coaches Expense Form below and attach your receipts of all completed registrations and training, then email it to the SYHA Coaches Coordinator (below). SYHA does not reimburse for apparel or equipment purchases unless pre-approved by the Board.

Eligible reimbursements:

• USA Hockey Registration
• CEP Level Training
• Age Specific Module
• Background Screening

Due Date: All requests for reimbursements must be submitted during the submission window of December 1-31.  Please fill out the expense form below and attach all receipts in the same email. Reimbursements will be sent out by February 1st.

IMPORTANT: Please include your mailing address and the name of the team(s) you are coaching. You must be currently registered to coach with SYHA and have a signed coach's agreement on file.
